Want to start mental health services? Please click on the appropriate link (adult, child, tcps only) to begin your paperwork.

You will also need to submit your insurance card via the dropbox labeled insurance.
if there is a custody agreement in place for a minor child a copy of this must also be submitted via the dropbox link labeled custody before services can begin.

Please note that completing the new client forms

does not guarantee that we will be able to provide you with services. We will review your request and contact you as soon as possible regarding the status.

If we are unable to offer services, we will do our best to provide you with referrals to other appropriate providers.

Adult
